PLEASE NOTE: During COVID-19 we are only taking in essential registrations.  If you live within 1 hours travelling time from your surgery we recommend you remain with them until lockdown is over. This is because your current surgery has full access to all your medical records and your health requirements.  They can issue medication requests electronically on your behalf and send the prescription to a pharmacy within the Maidenhead area.


If you have moved from an area more than 1 hour away from Maidenhead, we are happy for you to complete the necessary forms attached on the tabs above, as long as your address falls within our catchment area (please refer to the map).  Please post your applications through the surgery letterbox. The surgery is currently closed to protect you and our staff. We are only allowing entry to the surgery for essential nursing care.


Please ensure that you e mail copies of the required proof of address and photo ID to us and note on the e mail that the copies are for registration purposes rosemeadsurgery@nhs.net.  Please note, we reserve the right at a later date to ask you to attend the surgery with the originals of your documentation once lockdown is over.


Registering with us


Prospective patients moving into our catchment area may wish to register with the surgery.  You can click on the link to our catchment area map below, to ascertain if you are living within catchment and eligible to register with the surgery.  Registering with the surgery can take up to two weeks but in the meantime, your current surgery has a duty of care towards you whilst the registration process is taking place. When registering with the surgery it would help us if you have your NHS number (this can be found on your medical card) as this enables us to process your registration much quicker. 


You will also need to bring photographic proof of identity (driving licence or passport) and proof of residency (bank statement/ utility bill - which must be dated within the past 3 months) with you. We are unable to process your registration without these documents. If you are registering your whole family, we will need to see each member in person. This is an NHS requirement and is to prevent fraud.


Your New Patient Questionnaire will be reviewed by one of our practice nurses who may suggest you make an appointment for a health check if she feels it would be appropriate. If you are currently taking repeat medication, you must make an appointment with a GP as repeat medication will NOT be issued until the GP is satisfied that the medication and dosage is correct.
